| The Lake County Health Department does
have a Title X (Ten) vasectomy
program. Vasectomy services are provided on site by a local physician. |

While the Lake County Health
Department facilities in Umatilla and Tavares are undergoing
renovation, the main Health Department clinic is at 17580 US 441 in
Mount Dora.

The Clinic is on the south side of
the east-west stretch of US 441 north of Mt. Dora. It is 0.3 miles east of
a Wal-Mart Supercenter, 1.4 miles west of Donnelly Street and just west of
Morningside Drive.

Community Health
Centers, Inc.
Community Health Centers is one of two health
care providers in Florida (other than the Florida Department of Health and its
county health department clinics) which receives Federal Title X (Ten) Family
Planning funds directly from the regional office in Atlanta. Their Lake County
clinics are located in Leesburg (352) 360-0490 and in Groveland (352)
429-4104. Call either location and ask about obtaining vasectomy services.

Although he does not
perform vasectomies for the Lake
County Health Department, Dr. Doug Stein performs
vasectomies on a regular
basis at the Planned Parenthood in Orlando and at the
Cala Hills Family Clinic in Ocala for a fee of $390. For dates, see his "Outfield
Schedule". Medicaid and Medicaid HMO patients are accepted with
proper referral, but 2 visits are required to allow for the mandatory 30-day
interval (required by Medicaid) between signing your consent and having your
vasectomy.
