Comprehensive Pain
Management (CPM) office in Lake
City is one of Dr. Doug Stein's established
sites for vasectomy services. Dr.
Yili Zhou owns the
facility and is kind enough to allow Dr. Stein to use
it
periodically to make low-cost
vasectomy services available
to men in North Florida and South Georgia. Lake
City is less than one hour from Jacksonville and Valdosta and about
1.5 hours from Tallahassee. The fee is
$390, cash, check, Visa
or MasterCard. Friday

Page. To schedule, call Dr. Stein's office toll-free 866-VAS-TIME
(866-827-8463). Staff is available to take your call between 9:00 and
6:00, M-F. During other hours,
or if you get a recording because someone else is on the line, you may leave a

The CPM
office is located in Perimeter Park, the entry to which is on
State Road 47 just south
of downtown Lake City.

Directions:
From the east
(e.g. Jacksonville), take I-10 west to
Exit 303, At the end of the
exit ramp, zero your trip odometer as you turn left (south) onto
US 441 (Marion
Avenue). Drive 3.0 miles and turn right on NW Long Street. Drive
only 0.1 mile to NW Main Boulevard (US 41). Turn left and take that south to
the SR 47 cutoff (purple dot on map), at which point your odometer
will read 5.2 miles. If the detour is still in effect because of
construction, continue south on US-41 until your odometer reads 5.6
miles. Turn right on Bascom-Norris Drive
(Road), and at 5.7 miles, turn left (south) on State Road
47. At 6.1 miles on your trip odometer you will arrive at Perimeter
Park on the left side. The CPM office
is toward the back of the office park (it's very new and many lots
are still empty).
From the north
(e.g. Valdosta), take I-75 south to
Exit 427. Travel east on
US 90
0.8 miles to Bascom Norris Road. Turn right and follow
Bascom Norris
Road 2.4 miles to State Road 47. Turn right, travel 0.35 miles south
and Perimeter Park will be on the left side.
From the west
(e.g. Tallahassee), take I-10 east to
Exit 301, then
US 41 south
through downtown Lake City. Four and one-half miles south of
Exit
301, take the
right-hand fork onto State Road 47 (purple dot on map). From there, Perimeter Park is
0.8 miles south on the left side (if construction still requires a
detour, see above). The CWH office is toward the back
of the office park (it's very new and many lots are still empty).
Alternatively (and perhaps easier because it avoids downtown
traffic), take I-10 to
I-75 south and get off at
Exit 427. Go east
on US-90 0.8 miles to
Bascom Norris Road. Turn right and follow
Bascom Norris Road 2.4 miles to State Road 47. Turn right, travel
0.35 miles south and Perimeter Park will be on the left side.
From the south
(e.g. Gainesville), take I-75 north to
Exit 423. Take State Road 47
north 2.8 miles. Just beyond Michigan Street, you will see the sign
for Perimeter Park on the right side. The CPM office is toward the
back of the office park (it's very new and many lots are still
empty). |

When you arrive at the
Lake City location, you will be asked to provide demographic and health history
information as you are at any doctor's office. You may now register online
before your visit. The benefits are numerous and listed in our
On-line Registration Page.

Financial Aid
Medicaid and Medicaid HMO patients are accepted with
proper referral, but 2 visits are required to allow for the mandatory 30-day
interval (required by Medicaid) between signing your consent and having your
vasectomy.
Many private insurance plans cover vasectomy.
If you have insurance, call your carrier and ask the questions listed
here. Then call Dr. Stein's office
(813-972-1365 or 866-827-8463) to see if they can accept payment from your carrier.
If you have no health insurance (Medicaid or
private) and your income is low, you may
qualify for financial aid under a federal program
called Title 10, administered by many of Florida's county health
departments. The Columbia County Health Department has a Title 10 vasectomy program.
